Eldon Road Surgery is relocating to new Premises at 2-4 Alexandra Road, Reading, RG1 5PE from the 5th August 2024. Eldon Road Surgery will cease to operate from Thursday 25th July 2024. We will continue to have Melrose Surgery at 73 London Road. Please clarify where your appointment is at the time of booking. We hope that this move will help us to serve you better.

Fit Note

  • Fit notes (Commonly known as sick notes) are provided to sign patients off from work on health grounds. Please read the NHS rules on fit notes by clicking on the link below:

  • You need to be assessed by a health care professional before they could issue you with a fit note.

  • If you are not fit for work, you must self-certify yourself sick for the first week. Please click on the link below to generate a self-certification statutory sick pay form:

  • If you insist on a fit note for the first week, it can be issued by the health care professional at their discretion but will be deemed private and will incur a charge.

  • If you need an extension of a fit note, a fit note for a long-term condition that has worsened, or a new one, for a condition for which you have consulted a health care professional, please complete, and submit the form below:

  • The clinician can reject your request, if deemed inappropriate, or ask you to provide more information before a fit note could be issued.

  • Please give up to 72 hours to process a fit note.

  • We normally text a fit note as an attachment. If you need a paper copy, please mention in your request. You will need to collect it from our reception.
