(Commonly known as sick notes) are provided to sign patients off from work on

You need to be assessed by a health care professional before they could issue you with a fit note.
If you are not
fit for work, you must self-certify yourself sick for the first week. Please
click on the link below to generate a self-certification statutory sick pay

If you insist on a fit note for the first week, it can be issued by the health care professional at their discretion but will be deemed private and will incur a charge.
If you need an extension of a fit note, a fit note for a long-term condition that has worsened, or a new one, for a condition for which you have consulted a health care professional, please complete, and submit the form below:
The clinician can reject your request, if deemed inappropriate, or ask you to provide more information before a fit note could be issued.
Please give up to 72 hours to process a fit note.
We normally text a fit note as an attachment. If you need a paper copy, please mention in your request. You will need to collect it from our reception.
